The mouth and the 'tude are back.  Out. Of. Control.  I want to stop, I think?  Not sure.  It's like a reflex with all the sass and fire that goes with it. I've tried. Lord knows, I've tried.  My shrink and I have broken up.  He's moving.  I could still go through the end of the summer.  No point,  I can't have the kind of medicine I need anyway. Surrendering or submitting or whatever you want to call it... hard to do that when the person you want to lead you is completely content in his role as resident teenager. Sending the control freak packing... not really possible when the world comes screeching to a halt every time you relax and let go of control.  Even harder when help is offered to me, not followed through with, making twice as much work for me later, and then he has the audacity to shine his condescending little smirk and comments when I look frustrated about it. One can bite her tongue for only so long.  Which then serves as proof that I'm not trying to behave. It's been a time trying to reconcile things. He's not interested in bettering anything other than his job.   I despise the role I must live.  There I said it.  I thought I just wanted different.  Nope, I despise every bit of how I am and my role in our marriage. I'm tense. I'm angry. I'm anxious.  Thank goodness for xanax.  The ADHD monster has long since taken over my head again and I'm his pawn.  The OCD monster is taunting me, begging me to come back to the dark side.  Not sure what to do or where to turn anymore. My current thought is to wait until this current little tiff between us rolls over. It's hard to settle things when you both know you're in the right and the other is being an idiot.  For the record, I genuinely am right this time...  not that it matters lol There are a couple of things/routines that I know settle me and help keep the family in order.  Even though I know these things make a huge difference, I <strike>struggle to</strike> rarely do them.  I need to plan meals and make them - otherwise we eat out, waste money and stress the family out.  I need to keep the kitchen island and the little sink clean - otherwise the kitchen breeds clutter that stresses everybody out.  I need to make lunches and set out clothes the night before - otherwise mornings are stressful.  I need to go to bed on time - otherwise I'm a cranky bitch the next day.  I'm going to ask for his help in enforcing them. I don't know if he will, or how he will for that matter.  But maybe this is a way to work through my end of the problems. In the meantime... I'll just keep putting on my happy face and keep saving the world, as always.

I had come to know his wife in person, the children in his frequently exasperated anecdotes and his grandchildren in blurry cellphone photos. His wife left a message with the unexpected news on a Monday morning.It took me almost two weeks to get back to her.How typical, right? The doctor shrugs and moves on. The doctor forgets the symbolic value of closure. The doctor doesn't bother to show up at the funeral. The doctor ignores the obligations that endure after the patient passes.But hold your judgments. I called Lenny's wife back promptly, repeatedly, leaving increasingly puzzled messages and never getting a response.I suppose you could call it all a medical error. But it wasn't mine.Medicine has become a team sport — that's old news. Everyone knows that the team now includes residents, physician assistants, <a href="http://topics.nytimes.com/top/news/health/diseasesconditionsandhealthtopics/nursing_and_nurses/index.html?inline=nyt-classifier">nurse practitioners</a>, therapists, all taking over portions of the role that once belonged to the doctor alone.These colleagues slice and dice the doctor's time-honored obligations into fragments that can be difficult to reassemble. Residents routinely use my license number to write prescriptions; if the resident makes a big mistake, who is liable? Nurses inject medications per my orders; if I miscalculate a dose and disaster ensues, who pays?These days, that's not the end of it, not by a long shot. Behind the medical team stands an increasingly large backfield, giant in numbers, humble in training. They answer the phones, arrange consultations, organize the billing, deal with the unrelenting demands of dozens of insurers. Say that a secretary, a pleasant, overworked person sitting with her colleagues in an airless back office, each fielding dozens of calls a day — say she forgets to confirm a phone number scrawled into the medical record long ago. Who is responsible?You won't find the answer in the ethics textbooks. Experts continue to talk about the "physican-patient relationship," that storied historic and fiduciary entity, as if it were still composed of mutual trust and obligations between two people. They haven't noticed that those two people have turned into a crowd.Lenny was impossible, the best and the worst kind of patient. He never complained, mostly because he minimized half his problems and overlooked the rest. He vastly preferred to ignore his health and discuss his latest computer acquisitions. We spent a lot of office time on Apple vs. Microsoft. Whose fault was that?In the long run, it hardly matters. People change medications from good to not so good because their insurer pays for one set and not the other. People get their doctor on the phone right away because a secretary has the nerve to transfer the call to the staff lounge. People never hear from their doctor because a secretary forgets to confirm a phone number.After 10 days with no word from Lenny's wife, I was beside myself. I couldn't leave any more messages. I couldn't fathom why she wasn't calling me back. I figured she was angry with me. So, of course, I got a little angry about that. Does everything have to be my fault? Then I remembered that I knew where she got her own medical care, at an office bound to have more contact information.Sure enough: I had been calling the wrong number the whole time, off by one digit.Two conversations end this story. The first one, between me and our secretary, was brief. There wasn't much to say. I am not her supervisor. Mistakes happen. She didn't know Lenny; she seldom gets out from the back room to see our world — hers as much as mine — in all its dimensions.Then Lenny's wife and I talked for a long time. The edge in her voice eventually softened. We agreed that he had been impossible, and would be grievously missed. These summits are held all over the world very successfully by the Ed Tech Team from the USA, but this is a first for Africa. The summit aims at K-12 teachers and Higher Education. Like sands through the hourglass, so is the mantel in my living room. The sucker is a revolving door of indecision. As my dear friend <a href="http://www.callingithome.com/" target="_blank">Linda</a> says, I'm mantelly challenged. In fact, I've even got a <a href="http://pinterest.com/ladybaum/mantels/" target="_blank">board on Pinterest</a> just for mantels - it is an obsession. The bug hit again not too long ago and I changed it up after seeing what <a href="http://brynalexandra.blogspot.com/2013/02/honored.html" target="_blank">Bryn did with a client's mantel</a>. Our brothers and sisters in British Columbia are standing strong against IKEA, which locked them out eight weeks ago. The furniture giant made a concessionary contract offer that 84 percent of the workers rejected. David Halme writes in <a href="http://www.thestand.org/2013/07/ikea-lockout-aims-to-bust-union-in-b-c/">The Stand</a> that IKEA now wants to bust the union: <blockquote class="tr_bq">More than 300 workers at IKEA here have been locked out of their jobs since May 15 when they rejected a company offer that included the imposition of a tiered wage structure. Teamsters Local 213 were thrilled June 5 when the British Columbia Labour Relations Board found IKEA guilty of using scab workers to keep the Richmond store open. </blockquote><blockquote class="tr_bq">Even though IKEA profits worldwide have consistently increased, they have a lousy reputation beyond Swedish borders. The Pew Research Center came out with a new poll showing <a href="http://www.people-press.org/2013/06/27/favorable-views-of-business-labor-rebound/">that a majority of people have a favorable view of labor unions</a> -- 10 percent more than two years ago, the last time that Pew asked the question.  And over 60 percent of adults younger than 30 support unions – even if <a href="http://www.bls.gov/news.release/pdf/union2.pdf">fewer than 5 percent of them actually are unionized</a>.  So why is there so much support? Younger workers are increasingly introduced to a crappy economy with no safety net.  After the housing crisis, <a href="http://www.salon.com/2013/06/29/the_capital_of_exploitation_temp_town_usa_partner/">it has become harder and harder to find a full-time position</a>, since companies can get away with treating workers badly. Median pay has declined for workers, while corporate CEOs earn vast sums. Not only is America more unequal than ever, but <a href="http://thecontributor.com/daily-ceo-pay-now-exceeds-us-workers-annual-salary">the average CEO makes more in a day than an average worker makes in a year. From Occupy Oakland to Moral Mondays, workers have protested for a better economic and political system, but changes to government are long and arduous.  On the other hand, labor protests are typically short and to the point.  Contract negotiations, strikes, and other labor tactics get people better working conditions and decent jobs time after time.  In 2012, <a href="http://www.bls.gov/news.release/union2.nr0.htm">full-time union members made over $200 more than non-union full-time workers. </a> With those kinds of benefits, who wouldn't want to be part of a union?

Nothing at all. But the Dignity Memorial company, also known as <a href="http://www.integrityinillinois.com/">Houston-based SCI</a>, is asking striking funeral directors to scab. This is the same company that hired armed thugs six years ago to spy on Teamster funeral directors and drivers during negotiations. SCI could learn a thing or two about dignity. The striking funeral directors and drivers belong to Local 727 in Chicago. They walked off the job at 16 funeral homes a week ago because SCI refuses to negotiate in good faith. Here's what happened with the invitation to scab, according to the local's <a href="http://www.teamster.org/content/dignity-memorial-solicits-striking-employee-apply-funeral-director-job">press statement</a>: Lawrence Mandel was picketing in front of Dignity Memorial Piser Funeral Services in Skokie when he received an email: <blockquote class="tr_bq">We are contacting you for a potential opportunity on a temporary basis due to a strike in progress at some funeral homes owned and operated by SCI Illinois Services, Inc. These temporary assignments could lead to permanent positions based on the outcome of the strike and your performance; a permanent opportunity is not guaranteed... </blockquote><blockquote class="tr_bq">Thank you, Amber Kennedy SCI-Service Corporation International Recruiter </blockquote>John T. <div style='font-style:italic;' class='uawbyline'>By Kathy Kaufman</div> <div class='uawarticle'>Weight reduction surgery, also called bariatric surgery refers to a series of procedures carried out on overweight people. The size of the stomach is either reduced or a portion of it removed. This is possible either through use of a gastric band or sleeve gastrectomy respectively. Gastric bypass can also be done by resecting and diverting the small intestine to the stomach. Research has shown that this procedure helps in improving the condition of diabetic patients and reducing the risk of cardiovascular risks. The bariatric procedure is recommended for persons that have a minimum BMI of 40. It is also helpful in persons that have medical conditions that are exacerbated by excess weight. It is important to emphasize that this option should only be considered when all the conservative options have failed. The benefits and risks of the procedure should be considered in full before a decision is made as to whether or not to have it. Most common complications that should be considered are malabsorption syndromes and gall bladder diseases. There is a high risk of patients getting into depression and as such pre-operation counselling is warranted. Surgical procedures are grouped in to three main categories. These are malabsorptive, restrictive or mixed. The most commonly performed is the malabsorptive procedure. Malabsorptive operations aim at creating a physiological state of malabsorption. Biliopancreatic diversion combined with duodenal switch (BDS/DS) is one such undertaking. A section of the stomach is resected, creating a smaller pouch which is then directly connected to the ileum, bypassing the duodenum and jejunum. The malabsorption that results can be severe enough to cause serious deficiency diseases such as osteoporosis and anemia unless vitamins and mineral supplements are administered. Predominantly restrictive procedures aim at minimizing oral intake by restricting gastric volume. This produces early food satisfaction. Vertical banded gastroplasty involves permanent stapling of the stomach to create a small stomach to handle the food. It is regarded as one of the safest operations carried out today. In sleeve gastrectomy, the stomach is reduced to 85% its original size. A huge portion of the stomach following the greater curvature is removed. This makes the stomach look more like a tube or shaped like a banana. It is a permanent procedure done under laparoscopy. Although the volume of the stomach is reduced, it still works normally and the patient can be able to take in small amounts of food at a time. In the immediate post-operation period, one should only take liquid foods such as fruit juices and broth and gelatin desserts which are sugar-free. One should eat these foods until the gut fully recovers from the operation. Other diets to be introduced slowly include skimmed milk, cream soup, protein drinks and blended foods. Adverse effects of <A href="http://www.byyoursideweightlosstravel.com/bariatric-surgery">weight reduction surgery</A> are common. Due to reduced calcium absorption, patients can develop metabolic bone disease in form of secondary hyperparathyroidism and osteopenia. Rapid weight loss can also result in gallstones. In addition, reduced absorption of nutrients such as thiamine, folate, iron and vitamin B12 can lead to nutritional derangements.
