Cardiologist Eric Topol says he knew medicine had reached a turning point when patients started emailing him the results of do-it-yourself electrocardiograms.<br /><br><br /><br>With the help of a smartphone, a software application and a portable device that reads a person's heart rhythm, anyone can get an instant EKG reading on their phone screen.<br /><br><br /><br>"I am getting emails from people saying, 'I'm in atrial fibrillation—what do I do?' " Dr. Topol says, referring to a type of irregular heartbeat. "Whoa! The first time I saw that in the subject line of an email, I said, the world has really changed."<br /><br><br /><br>Mobile apps for smartphones and tablets are changing the way doctors and patients approach health care. Many are designed for the doctors themselves, ranging from handy databases about drugs and diseases to sophisticated monitors that read a person's blood pressure, glucose levels or asthma symptoms. Others are for the patients—at their doctor's recommendation—to gather diagnostic data, for example, or simply to help coordinate care, giving patients an easy way to keep track of their conditions and treatments.<br /><br><br /><br>Doctors say many of the apps are useful time savers, and have the potential to make health care more efficient by speeding diagnosis, improving patient monitoring and reducing unnecessary visits to a physician or hospital. The Biggest Mistake Doctors Make - WSJ.com

A patient with abdominal pain dies from a ruptured appendix after a doctor fails to do a complete physical exam. A biopsy comes back positive for prostate cancer, but no one follows up when the lab result gets misplaced. A child's fever and rash are diagnosed as a viral illness, but they turn out to be a much more serious case of bacterial meningitis.<br /><br><br /><br>Such devastating errors lead to permanent damage or death for as many as 160,000 patients each year, according to researchers at Johns Hopkins University. Not only are diagnostic problems more common than other medical mistakes—and more likely to harm patients—but they're also the leading cause of malpractice claims, accounting for 35% of nearly $39 billion in payouts in the U.S. from 1986 to 2010, measured in 2011 dollars, according to Johns Hopkins.<br /><br><br /><br>The good news is that diagnostic errors are more likely to be preventable than other medical mistakes. And now health-care providers are turning to a number of innovative strategies to fix the complex web of errors, biases and oversights that stymie the quest for the right diagnosis.<br /><br><br /><br>Part of the solution is automation—using computers to sift through medical records to look for potential bad calls, or to prompt doctors to follow up on red-flag test results. Another component is devices and tests that help doctors identify diseases and conditions more accurately, and online services that give doctors suggestions when they aren't sure what they're dealing with.<br /><br><br /><br>"Diagnostic error is probably the biggest patient-safety issue we face in health care, and it is finally getting on the radar of the patient quality and safety movement," says Mark Graber, a longtime Veterans Administration physician and a fellow at the nonprofit research group RTI International.Finally, there's a push to change the very culture of medicine. Doctors are being trained not to latch onto one diagnosis and stick with it no matter what. Instead, they're being taught to keep an open mind when confronted with conflicting evidence and opinion.<br /><br><br /><br>The effort will get a big boost under the new health-care law, which requires multiple providers to coordinate care—and help prevent key information like test results from slipping through the cracks and make sure that patients follow through with referrals to specialists.<br /><br><br /><br>There are other large-scale efforts in the works. The Institute of Medicine, a federal advisory body, has agreed to undertake a $1 million study of the impact of diagnostic errors on health care in the U.S.<br /><br><br /><br>In addition, the Society to Improve Diagnosis in Medicine, which Dr. Graber founded two years ago, is working with health-care accreditation groups and safety organizations to develop methods to identify and measure diagnostic errors, which often aren't revealed unless there is a lawsuit. In addition, it's developing a medical-school curriculum to help trainees improve diagnostic skills and assess their competency.<br /><br><br /><br>Robert Wachter, associate chairman of the department of medicine at the University of California, San Francisco, says defining and measuring diagnostic errors is an important step. "Right now, none of the incentives for improvement in health care are based on whether the doctor made the correct diagnosis," Dr. Wachter says. But equally important, he adds, "we need to nurture bottom-up innovation."<br /><br><br /><br>That's already happening. Large health-care systems are mining their electronic records for missed signals. At the Southern California Permanente Medical Group, part of managed-care giant Kaiser Permanente, a "Safety Net" program periodically surveys its database of 3.6 million members to catch lab results and other data that might fall through the cracks.<br /><br><br /><br>In one of the first uses of the system, a case manager reviewed 8,076 patients with abnormal PSA test results for prostate cancer, and more than 2,200 patients had follow-up biopsies. From 2006 to 2009, 745 cancers were diagnosed among those patients—and Kaiser had no malpractice claims related to missed PSA tests.<br /><br><br /><br>The program is also being used to find patients with undiagnosed kidney disease, which is often found via an abnormal test result for creatinine, which should be repeated within 90 days. From 2007 to 2012, the system found 7,218 lab orders placed for patients with an abnormal test that had not been repeated. Of those, 3,465 were repeated within 90 days of a notice to patients that they needed a repeat test, and 1,768 showed abnormal results. The majority, 1,624, turned out to be new cases of the disease.<br /><br><br /><br>Michael Kanter, regional medical director of quality and clinical analysis, says the system enables clinicians to go back "as far as is feasible to find all of the errors that we can and fix them."<br /><br><br /><br>Because the disease is slow moving, Dr. Kanter says, people with a five-year-old undiagnosed case may not have been harmed. Likewise, with many early prostate cancers, "in many of these cases it doesn't mean harm would have reached the patient," he says. "But we don't want patients not to have the information they should have had through some kind of lapse in the system."<br /><br><br /><br>Dealing With the Flood<br /><br>Electronic records aren't a panacea, of course, and can even lead to information overload. In a survey of Veterans Administration primary-care practitioners reported last March in JAMA Internal Medicine, more than two-thirds reported receiving more patient-care-related alerts than they could effectively manage—making it possible for them to miss abnormal test results.<br /><br><br /><br>Some researchers suggest the best solution isn't to flood doctors with information but to provide a second set of eyes to find things they may have missed.<br /><br><br /><br>The focus now is preventing dangerous delays in follow-ups of abnormal test results. In a pilot program, researchers at the Houston VA developed "trigger" queries—a set of rules—to electronically identify medical records of patients with potential delays in prostate and colorectal cancer evaluation and diagnosis. Records included charts that had no documented follow-up for abnormal findings suspicious for cancer after a certain period, according to the research team's leader, Hardeep Singh, chief of health policy and quality at Michael E. DeBakey VA Medical Center in Houston and an assistant professor of medicine at Baylor College of Medicine.<br /><br><br /><br>The queries were run on nearly 600,000 records of patients seen at one VA facility in 2009 and 2010. Dr. Singh says the use of triggers, which helped find abnormal PSA tests and positive fecal occult blood tests, could detect an estimated 1,048 instances of delayed or missed follow-up of abnormal findings annually and 47 high-grade cancers.<br /><br><br /><br>The VA has funded a randomized trial to test whether an automated surveillance system of triggers can improve timely diagnosis and follow-up for five common cancers.<br /><br><br /><br>"This program is like finding needles in a haystack, and we use information technology to make the haystack smaller and smaller so it's easier to find the needles," Dr. Singh says.<br /><br><br /><br>More health-care systems are also turning to electronic decision-support programs that help doctors rank possible diagnoses by likelihood based on symptoms and notes in the medical record. In a study of one such system, called Isabel, researchers led by Dr. Graber found that it provided the correct diagnosis 96% of the time when key clinical features from 50 challenging cases reported in the New England Journal of Medicine were entered into the system. The American Board of Internal Medicine is studying how Isabel could be linked to assessments of physician skill and knowledge.<br /><br><br /><br>Another system, DXplain, developed at Massachusetts General Hospital in Boston, was shown in a study last year to significantly improve diagnostic accuracy among first-year medical residents.<br /><br><br /><br>Edward Hoffer, associate clinical professor at Harvard and senior computer scientist at Mass General who leads the DXplain program, says the aim now is to have DXplain "push" diagnostic suggestions to physicians through an electronic-medical-records system rather than requiring doctors to initiate a query, which some are still reluctant to do. "We have to focus our attention on dealing with situations where doctors think they know what the diagnosis is, but they don't," Dr. Hoffer says.<br /><br><br /><br>Other Avenues<br /><br>New devices also hold promise for confirming a diagnosis and avoiding unnecessary tests. A number of companies are rushing to provide aids such as portable diagnostic equipment and lab tests that can analyze tiny samples of blood and other bodily fluids quickly to detect disease.<br /><br><br /><br>Consider MelaFind, which came to market in the U.S. in 2011. The device allows dermatologists to noninvasively examine moles as deep as 2.5 millimeters beneath the surface to gauge the level of "disorganization," an indicator of irregular growth patterns that are a sign of melanoma, among the deadliest cancers.<br /><br><br /><br>New York dermatologist Macrene Alexiades-Armenakas says she uses MelaFind to confirm that a mole is to be removed and prioritize the level of disorganization in multiple abnormal moles. In some cases, when another doctor or the patient has been concerned about a mole, MelaFind supported "clinical diagnosis of a benign mole, thereby sparing them a biopsy," she says.<br /><br><br /><br>But such devices will never replace a thorough physical exam with a trained eye and careful follow-up, says Dr. Alexiades-Armenakas: "These diagnostic tools are aids to increase our accuracy and adjuncts to good physical diagnosis, not a substitute."<br /><br><br /><br>Some efforts to cut down on errors take a different route altogether—and try to improve diagnoses by improving communication.<br /><br><br /><br>For instance, there's a push to get patients more engaged in the diagnostic process, by encouraging them to speak up about their symptoms and ask the doctor, "What else could this be?" At Kaiser Permanente, a pilot program provides patients with a pamphlet that encourages them to think about and write down their symptoms and what concerns or fears they have, encouraging them to ask specific questions to be sure they understand their diagnosis and the next steps they must take.<br /><br><br /><br>Medical schools, meanwhile, are teaching doctors to be more receptive to patient input and avoid "anchoring," the habit of focusing on one diagnosis and excluding other possible scenarios, and "premature closure," not even considering the correct diagnosis as a possibility.<br /><br><br /><br>The Critical Thinking program at Dalhousie University in Halifax, Nova Scotia, established last year, aims to help trainees step back and examine how biases may affect their thinking. Developed by Pat Croskerry, a physician known for his research on the role of cognitive error in diagnosis, it uses a list of 50 different types of bias that may lead to diagnostic error.<br /><br><br /><br>The program is being integrated throughout four years of the medical school. Students study cases such as a psychiatric patient with shortness of breath who was assumed to be merely having an anxiety attack; doctors overlooked that she was a smoker on birth-control pills, a risk for the blood clot that later traveled to her lung and killed her.<br /><br><br /><br>"If we can teach physicians how to think more critically," Dr. Croskerry says, "they would be more effective in delivering good care and arriving at the right diagnosis."<br /><br><br /><br><a href="http://online.wsj.com/news/articles/SB10001424052702304402104579151232421802264#printMode">http://online.wsj.com/news/articles/SB10001424052702304402104579151232421802264#printMode</a>

Google's New Tools Show How Deep Glass Will Embed in Our Lives

<div xmlns='http://www.w3.org/1999/xhtml'><p>The promise of wearable computers is that the devices themselves will go away. They'll melt into the background to deliver data as needed, all without your having to fetch or look it up. Google announced a few new features to its Glass software developers kit on Tuesday that give its face computer a greater ability to both be there and go away. With a few simple third-party apps, the company showed (probably better than ever before) just how Glass will evolve into something that's invisibly integrated into your day-to-day routine.</p><br /><p>Glass' new GDK - or Glassware development kit - makes it easier for application developers to tap into the hardware, even when its offline, to perform tasks in real time without having to go back and forth to servers in the cloud. It opens new possibilities for developers to build tools that center around either immersive or ongoing tasks (as opposed to, say, simply notifications about things like new email, or the ability to upload a photo and share it online).</p><br /><p>Immersive tasks will let you dive into an application on Glass, much as you might on a smartphone, giving it your undivided attention - think gaming, for example. An ongoing task is one where you're swapping back and forth between the app and something you're doing - think cooking. Google showed a few new apps from Strava, AllTheCooks, GolfSight, Word Lens, and Spellista meant to demonstrate how developers can now use Glass to deliver immersive experiences, or enhance ongoing ones.</p><br /><p>Spellista and Word Lens were immersive apps. The former is a simple letter jumble game and honestly it was ho-hum. Word Lens, however, was pretty amazing. It fires up with a simple voice command, 'Okay, Glass, translate this.' Then you can look at a sign written in a foreign language, and the app will automatically translate the text and overlay it on the display. This happens nearly instantly. It was straight out of the future. But the ongoing tasks were probably even more interesting than the immersive ones.</p><br /><p>Take the new Strava app for Glass. Strava, a tool for tracking bike rides or runs, is immensely popular with cyclists and gaining ground with runners. But it's the type of app that clearly benefits from not having to pull a phone from a pocket or bag to get an update - you don't want to have to pluck it from a pocket while you're in the bike lane on Market Street. The Glass app lets it deliver real-time updates on speed and distance, and deliver ride segment reports simply by letting you look up. It promises to help you be more situationally aware, more in the moment, while still getting data delivered when you want it. Similarly, Golf Sight can let you know where you are on a golf course, how far you are from the green, and what hazards are between you and it without having to dive into a phone.</p><br /><p>Or take AllTheCooks, which lets you view or record recipes using Glass. One of the other promises Glass has, so far, truly failed to deliver on is the ability to do something else with your hands while still getting data updates from the device. There's been much speculation how Glass may one day be used in the operating room. But AllTheCooks shows how remarkably useful it can be in the kitchen. You can simply play the steps back without taking your attention away from what you're doing. You don't have to worry about greasy fingers on a tablet, or printed page. Meanwhile filming step-by-step instructions without using your hands, promises to be something that all manner of apps will be able to take advantage of.</p><br /><p>When people have asked me about Glass, I've often tried to explain it by saying, 'it's Google Now for your face,' (a phrase I stole from Metafilter founder Matt Haughey). That's because the Google Now functions -delivering information to you before you ask for it - have been its killer feature, even more than the ability to take photos or videos. The way it can drop an announcement on you based on where and when you are is amazing. But previously I haven't seen third party applications tap into that same functionality. Instead, you get things like breaking news alerts from the New York Times, or notifications about @ replies on Twitter. But that's clearly changing.</p><br /><p>There's been a huge over-emphasis on what Glass is (aesthetically ugly, of limited utility, socially awkward) rather than what it will be. Today we got a glimpse of the path it's heading on, maybe our best yet. What's coming is a more technologically robust device that will enhance our situational and possibly even social awareness. (WTVR) – Virginia Senator Creigh Deeds is being treated for “serious injuries” after he was stabbed “multiple times” inside his Bath County home, according to Virginia State Police spokeswoman Corinne Geller.<br /><br />“After the stabbing, Senator Deeds left the scene on foot and walked down the hill of his residence to Route 42,” according to Virginia State Police. “The injured senator was spotted and picked up by a cousin who was driving along Route 42. The two drove to the cousin’s residence where the 911 call was placed to the sheriff’s office.”<br /><br />Deeds was flown to the University of Virginia Medical Center where his condition was not released. The senator’s son, 24-year-old Gus Deeds, died in the home from a gunshot wound, Geller said during a Tuesday afternoon press conference.<br /><br /><br /><br />Virginia State Police were called to Sen. Deeds’ Millboro-area home about 7:25 a.m. Tuesday.<br /><br />“Troopers arrived to find Sen. Deeds stabbed multiple times about the head and upper torso,” Geller said. “Senator Deeds’ son, Austin “Gus” Deeds, was found inside the residence, suffering from life-threatening injuries associated with a gunshot wound. Despite efforts by trooper and first responders, he died at the scene.”<br /><br />The investigation into the motive and sequence of events regarding Tuesday’s violence remained under investigation, Geller said. She added it was not yet known if Gus Deeds died from a self-inflicted gunshot wound, however, she said police were not currently looking for a suspect in the shooting.<br /><br />Sen. Mike Peifer believes northeastern Pennsylvania has become "infested" with coyotes that are preying on everything from house cats to deer.<br /><br />Peifer, R-139, is calling on Pennsylvania hunters to help cull the population. He's introduced legislation that would allow the state Game Commission to offer $25 for each coyote killed.<br /><br />The House is set to vote on the bill today.<br /><br />The bill wouldn't require the Game Commission to implement the incentive, but it would give it the flexibility to do so in areas where coyotes are a problem, Peifer said. State law already allows for coyote hunting year-round.<br /><br />"We're just trying to control this population a little bit," he said. "Let's give (hunters) an incentive to hunt coyotes."<br /><br />Peifer says the coyote problem first hit home on a bear hunting trip last year, when his party shot four coyotes. Nokia Shareholders Vote to Sell Cellphone Unit to Microsoft

<div xmlns='http://www.w3.org/1999/xhtml'><img src="http://graphics8.nytimes.com/images/2013/11/20/technology/20nokia-span/20nokia-span-articleLarge.jpg" width="600"/><br /><p>HELSINKI - Nokia's shareholders took a major step on Tuesday to reshape the company, voting overwhelming in favor of selling the company's handset business to Microsoft for $7.2 billion.</p><br /><p>The approval represents one of the final steps before Microsoft takes control of the struggling cellphone division, which is expected early next year. But it also leaves many questions about what is in store for Nokia, a Finnish company that still lies at the heart of Europe's technology industry.</p><br /><p>'It's been a very challenging period,' Risto Siilasmaa, Nokia's chairman and interim chief executive, said in a recent interview. 'When I became chairman, I didn't think this was going to be a possibility. There have been a lot of emotions.'</p><br /><p>The handset unit - which sold 64.6 million phones in the third quarter of 2013 - is still one of the world's largest. But it has increasingly lost out to rivals like Samsung and Apple that now dominate the lucrative smartphone market.</p><br /><p>After pioneering mobile phone technology throughout the 1990s, Nokia became the world's largest handset maker with a market value of around $250 billion. It had nearly a 40 percent share of the global smartphone market as late as 2009, according to Gartner, the research company.</p><br /><p>But Nokia's market share has quickly dwindled to less than 4 percent of global smartphone sales, and Samsung is the world's largest phone maker. The company's market value stands at roughly $30 billion, or just 12 percent of its record high.</p><br /><p>'It's a sad moment, but we have to look forward,' said Michael Halbherr, who leads Here, Nokia's mapping business, which will remain part of the company. 'The type of culture we had at Nokia created success, but it didn't do enough to succeed in the smartphone era.'</p><br /><p>Despite the company's recent lackluster performance in the cellphone market, Nokia still has a few tricks up its sleeve as it contemplates the future. Senior executives at the company are completing a strategy, due early next year, that is expected to focus on Nokia's remaining businesses and be supported, at least in part, by the cash from the handset sale.</p><br /><p>The plans are likely to include a networking unit that competes with companies like Ericsson of Sweden and Huawei of China to build high-speed mobile data infrastructure for large carriers, including Verizon Wireless and Vodafone. While the business began as a joint venture with Siemens, Nokia bought the 50 percent that it did not already own for $2.2 billion earlier this year.</p><br /><p>The company has also held on to its mapping division, whose technology is in about 80 percent of the automotive navigation systems worldwide. It also controls an intellectual property portfolio that could either be licensed to other companies or used to build new products.</p><br /><p>'We are in a unique situation,' Henry Tirri, Nokia's chief technology officer, who manages the company's intellectual property, said in an interview. 'Before, we only thought about fitting our technology into mobile devices because that was our main business. Now, we have kept our technological assets and can use them where we see opportunities.'</p><br /><p>Analysts warn, however, that creating a viable business from the three separate divisions, which are run independently, could prove a challenge.</p><br /><p>Nokia's networking division, for example, will generate over 90 percent of the company's revenue after the handset sale is completed. With little overlap with the company's mapping unit, some investors have called on the firm to sell, or spin off, the smaller unit so that Nokia can prioritize its core networking operations.</p><br /><p>'These are very different business,' said Sylvain Fabre, a Gartner telecoms analyst in London. 'It's hard to see how they could create connections between them.'</p><br /><p>Other shareholders have demanded that Nokia return some of the almost 8 billion euros, or $10.8 billion, in cash that Nokia will have after it completes the sale of its handset division.</p><br /><p>Analysts say that a portion of the proceeds will probably be spent on acquisitions, particularly for Nokia's mobile networking business, but that some of Microsoft's money also will make its way to wary investors, who remain unsure of Nokia's future plans.</p><br /><p>'We expect a meaningful portion of the excess will be distributed to shareholders in coming quarters,' the American hedge fund manager Daniel Loeb wrote in a letter to his investors last month.</p><br /><p>While investors bicker over how the money should be spent, Nokia's executives and employees - and many Finns - are still coming to terms with the new-look company that has been synonymous with handsets for more than two decades.</p><br /><p>Many in the Northern European country viewed Nokia's position as a leader in the cellphone industry as a matter of national pride, and the company helped to train software engineers and developers who transformed Helsinki into one of Europe's leading technology hubs.</p><br /><p>The sale is also the first time in the lives of younger generations in both Finland and around the world that Nokia will not be known for making handsets. Analysts and investors have raised concerns about how Nokia will be able to connect with global consumers who still associate the brand with mobile phones. Microsoft has a 10-year licensing deal to use the Nokia brand for some of its own phones.</p><br /><p>Rajeev Suri, head of the company's network business, says his priority is now squarely on increasing Nokia's market share in so-called fourth-generation mobile infrastructure, which supplies high-speed cellular connections. The business has been growing as consumers increasingly rely on smartphones and tablets to connect to the Internet.</p><br /><p>Mr. Halbherr of Nokia's mapping unit also wants to extend the company's foothold in GPS navigation by building relationships with some of the world's largest automakers, which already use the firm's products and services.</p><br /><p>Still, for others, the handset sale remains a hard pill to swallow.</p><br /><p>Janne Lindström joined Nokia's networking unit in the mid-1990s, and has worked in a number of manufacturing jobs alongside his counterparts in the cellphone business at one of the company's plants in northern Finland.</p><br /><p>Despite Nokia's recent problems, Mr. Lindström said he had been hopeful that the company's networking business, which itself underwent a restructuring that involved 17,000 job cuts worldwide, would help to support the faltering handset business so that it could remain part of Nokia.</p><br /><p>'The announcement with Microsoft took me by surprise,' he said. Another New Season of Weight Loss

<a href="http://www.enjoyingthislife.com/" target="_blank">Patty</a> really sparked an idea for me with her comment on last week's post.  She suggested starting with just clean eating - no processed foods - and let it be as that.  That was a great suggestion and really got my wheels turning.<br /><br />When I think back to all the years I've done Weight Watchers (literally, since I was in high school during the days of the paper Points slider), I tried to reflect on what worked for me in each segment.<br /><br />High school was more challenging staying on program, since I had to rely mostly on my parents to pick my food choices.  Additionally, there was so much temptation at school.  How could I pass up pizza and cookies when all my friends could freely eat that?<br /><br />In college, however, I really hit my WW stride.  What worked for me at that time was consistent exercise (7 days/week) and eating virtually the same thing day in and day out - lots of Lean Cuisine and Smart Ones meals.  It was easy and almost thoughtless.  I just got into the freezer, tore open that box and dinner was served.  Then, after about a year of that, it all got boring and monotonous.  I got sick of Lean Cuisine (and still, to this day, cannot handle one of those frozen meals!) and burned out on daily exercise.  But, that lifestyle worked for a year, contributing to a 50 lbs weight loss, so I was doing something right, just not something I could maintain forever.<br /><br />Then I moved to DC and my progress ebbed and flowed here, and eventually I gained back those 50 lbs lost, plus some.<br /><br />Before I got pregnant, I was down around 30-40 lbs from my highest weight (263.5 lbs).  And what worked during that season of weight loss seemed to be small steps and small goals.  During that season: I really got into TBW (the writing helped keep me focused), and I slowly added exercise back into my life (consistently exercising each week for at least 3 days/45 min at a time - totally manageable!).  Still, I was starting to struggle right around September 2012.  Some of those struggles were surrounding poor planning.  And some of those struggles were surrounding blogger impressions - feeling like a weight loss blogger failure - not putting out content that people wanted to read, etc etc.  My focus was definitely off, especially because I didn't start this blog for readers, I started it for me. <br /><br />The thing is - life right now is overwhelming.  Trying to "do it all" isn't doing me any good.  I struggle with managing working full time, keeping up the house (laundry, cleaning, dishes), cooking, taking care of the baby and finding time for myself and my husband.  Most days I can't even tell you what I did - the day just comes and goes and all I feel is a headache by the day's end. <br /><br />So, I think I need to start small again.  And by small, I mean miniature!  I remember when I first went back to WW in July, I read somewhere how to start with committing each week to doing something new.  Even if it's as simple as - "this week I will track just breakfast" - then each week I'll add on to that. <br /><br />It almost seems reckless just tracking one meal, but honestly, I think that will do me a lot more good than what I've been doing.  I have to try everything.  I have to exhaust every option until I find something that works for me and this new season.<br /><br />I cannot remain this way forever, both in weight and in eating behaviors.

Rick Nash returns to New York Rangers; Buffalo Sabres shuffle roster

<div xmlns='http://www.w3.org/1999/xhtml'><img src="http://i2.cdn.turner.com/si/dam/assets/131119133952-rick-nash-2-single-image-cut.jpg" width="600"/><br /><p>NEW YORK (AP) -- Rick Nash is back in the New York Rangers lineup after the star forward missed 17 games because of a concussion.</p><br /><p>Nash passed his final test Tuesday when he skated with his teammates before the game that night against the Boston Bruins. Nash has being skating - mostly on his own - the past two weeks after his symptoms subsided.</p><br /><p>He was cleared to return Sunday and told coach Alain Vigneault the next day he was ready to go. Nash hasn't played in a game since he was injured on a hit from San Jose's Brad Stuart on Oct. 8 in New York's third game of the season.</p><br /><p>'I'm going to go tonight,' Nash said after the morning skate at Madison Square Garden. 'Last night, I just thought it would be the best thing, moving forward, talking to everybody. I'm excited, it's kind of controlled emotion. It's been a long six weeks so I'm excited to get in there.</p><br /><p>'I think the first everything is going to take awhile, whether it's a hit, a shot or a pass, making different plays. It's a long time off, so it's going to take some time.'</p><br /><p>Vigneault plans to use Nash as he normally would before the injury - his second concussion in eight months - but he will talk to him throughout the game to see how he's feeling.</p><br /><p>'He is going to play a regular shift and he is going to play some power play,' Vigneault said. 'I would like to be able to keep him in that 15-minute range. He hasn't played for quite some time. It's almost for him like going through training camp again.'</p><br /><p>Nash skated for several consecutive days and then practiced with the team last week, first in a noncontact capacity. He underwent tests Thursday.</p><br /><p>Nash logged just under six minutes of ice time the night he was hurt in the first period. It was the only game of the three he played this season in which he failed to record a point.</p><br /><p>He received just more than 15 minutes of ice time in the season opener and just under 18 minutes in the second game - one day before he was injured.</p><br /><p>'I think we'll just guide it as we go along,' Nash said. 'I'm sure (Vigneault) has had a lot of time with players coming back from injuries, so I leave that up to him.'</p><br /><p>J.T. Miller will be a healthy scratch from Tuesday night's lineup. The Rangers had already sent down forward Brandon Mashinter to Hartford of the AHL on Monday.</p><br />Sabres return D Zadorov to OHL; demote 3 to AHL<br /><p>BUFFALO, N.Y. (AP) -- The Buffalo Sabres have put their youth movement on hold a week after Pat LaFontaine took over.</p><br /><p>LaFontaine, the team's new president of hockey operations, announced a significant roster shuffle on Tuesday, when the Sabres demoted four rookies before their home game against St. Louis.</p><br /><p>Veteran defenseman Mike Weber also was activated from injured reserve after missing 10 games with a thumb injury and is expected to play against the Blues.</p><br /><p>Defenseman Nikita Zadorov was returned to his Canadian Junior team, the Ontario Hockey League London Knights. The move was made before Zadorov played his 10th NHL game, which is when his three-year contract would come into effect. Drafted 16th overall in June, Zadorov had a goal in seven games in Buffalo.</p><br /><p>Fellow 2013 first-round pick, defenseman Rasmus Ristolainen, and forward Johan Larsson were assigned to the Sabres' AHL affiliate in Rochester, N.Y.</p><br /><p>Center Mikhail Grigorenko is also heading to Rochester but on a conditioning assignment. That move gives the Sabres two weeks to determine whether to recall Grigorenko or send him to his junior team in Quebec.</p><br /><p>Grigorenko has struggled finding a niche in Buffalo since being selected in the first round of the 2012 draft. After making the Sabres roster to start last season, Grigorenko was returned to Quebec in March.</p><br /><p>He made the Sabres once again this season, but has lacked playing time. Grigorenko has missed five of the past seven games and has two goals and an assist in 15 games this season.</p><br /><p>The demotions mark a significant shift in philosophy under LaFontaine and interim coach Ted Nolan. The two took over after general manager Darcy Regier and coach Ron Rolston were fired last week after the Sabres got off to a franchise-worst 4-15-1 start.</p><br /><p>Regier made it clear that he was intent on rebuilding the Sabres through younger players, no matter the team's record.</p><br /><p>On Tuesday, Nolan said the Sabres intend to fill the open roster spots later this week by recalling players from Rochester. Giants revival the latest twist in the always unpredictable NFC East

<div xmlns='http://www.w3.org/1999/xhtml'><img src="http://i2.cdn.turner.com/si/dam/assets/131119132227-new-york-giants-nfc-east-single-image-cut.jpg" width="600"/><br /><p>I'm starting to think the New York Giants have the rest of the NFC East right where they want them. This is a division that has a rich recent history of being Curveball Central in the NFL, giving us a playoff team that we never saw coming, and you have to wonder if it's happening again. Are the once-left-for-dead Giants in the process of becoming the first team to ever start 0-6 and still make the playoffs? Did I really just write that sentence?</p><br /><p>All I know is something weird and unexpected seems to happen in the NFC East playoff race on a regular basis, and the Giants, winners of four games in a row, suddenly own the division's longest winning streak of the season. If Tom Coughlin's resurgent team beats the faltering Cowboys (5-5) at home on Sunday, it will stand 5-6, just one game behind first-place Philadelphia (6-5, on a Week 12 bye) with five games remaining.</p><br /><p>That's right. One game back with five to play, with the Giants' only remaining division games at that point being two meetings with last-place Washington (3-7), a team that looks ready to really unravel in the final six weeks. That we're even considering the playoff possibilities of a Giants team that has been outscored 256-192 this season, with 29 turnovers, is improbable enough. But that's the state of things in this year's NFC East, and really, late-season strange has been the division's default setting since roughly 2005.</p><br /><p>A brief history lesson of the weirdness is in order:</p><br /><p>* 2005 -- Washington starts the season 5-6 and is tied for last place with Philadelphia after Week 12. But Joe Gibbs' team catches fire down the stretch, winning five in a row -- including the final three against division rivals Dallas, the Giants and Philadelphia -- to finish 10-6 and claim the NFC's second wild-card berth.</p><br /><p>* 2006 -- Philadelphia starts 5-6 that year, losing No. 1 quarterback Donovan McNabb to a season-ending knee injury in a Week 11 loss to Tennessee. But with Jeff Garcia taking over at quarterback, the Eagles go 5-0 in December, winning three consecutive division road games in Weeks 14-15-16. At 10-6, Philadelphia claims the NFC East title, beating out Dallas (9-7) after the Cowboys had started 8-4, and the Giants (8-8), who had once been 6-2.</p><br /><p>* 2007 -- The Cowboys cruised to a 13-3 record and the No. 1 seed in the NFC that season, but of course it was the No. 5-seeded Giants (10-6) who got to celebrate, storming to an unlikely Super Bowl title on the strength of three consecutive road playoff wins and that memorable upset of the vaunted 18-0 Patriots in Arizona. The weirdness really began with New York's inspiring 38-35 Week 17 <em> loss to visiting New England, a defeat that somehow set the Giants up for greater things in the weeks ahead.</p><br /><p>* 2008 -- After 12 weeks of the regular season, the Eagles were 5-5-1 and in last place in the NFC East, 4½ games behind the first-place Giants (10-1). But the Eagles closed fast once again, winning four of their last five games, including a huge Week 14 upset of the Giants in the Meadowlands. Philadelphia (9-6-1) made the playoffs as the NFC's No. 6 seed, beat the top-seeded Giants and third-seeded Vikings on the road in the playoffs, and weren't stopped until a defeat by Arizona in the NFC Championship Game.</p><br /><p>* 2010 -- The entire division race came down to one play that season: DeSean Jackson's game-winning 65-yard punt return as time expired in Philadelphia's ridiculous 38-31 comeback win at the Giants in Week 15. The Eagles trailed 31-10 with 8:17 left in regulation, but somehow beat New York for a sixth consecutive time. Philadelphia finished 10-6 and won the East. The Giants finished 10-6 and missed the playoffs despite starting 6-2, and later sitting 9-4.</p><br /><p>* 2011 -- Much like the current season, the division was a bastion of mediocrity for most of 2011. The visiting Giants were humiliated by New Orleans in a 49-24 Week 12 loss on Monday Night Football, and then came home to drop a 38-35 showdown with the undefeated Packers. Again, the near-miss against a perfect team sparked something for the G-Men, because New York went 3-1 down the stretch and beat Dallas in Week 17 to claim an unlikely division title at 9-7, becoming the first NFC East champion with fewer than 10 victories. Another four wins in the playoffs, including at top-seeded Green Bay and No. 2-seeded San Francisco, and the Giants had another improbable Super Bowl trophy.</p><br /><p>* 2012 -- Washington's seven-game season-ending winning streak was predicted by absolutely no one. Even Redskins head coach Mike Shanahan talked about starting to evaluate his players for 2013 when Washington lost at home to Carolina in Week 9, falling into sole possession of last place in the division at 3-6. But after a Week 10 bye, Washington was unbeatable, winning out to earn its first NFC East championship since 1999. The Redskins scored 27 points or more in six of their final seven games, going 5-0 against division rivals during that run.</p><br /><p>It's quite a track record of the unforeseen, and there's no other division in the league that can match it. So who cares if this season's Giants rank 28th in the NFL in points with 192, and have a woeful minus-11 turnover ratio that's tied for second-worst in the league (29 giveaways, 18 takeaways)? This is the NFC East, where anything can happen, and usually does.</p><br /><p>You can't overlook the rally the Eagles have mounted themselves, with a three-game winning streak that has vaulted them into first place at 6-5. But underestimating the Giants has proven to be a miscalculation when it comes to the NFC playoff picture, as history bears out.</p><br /><p>New York hasn't lost since Oct. 10, and the Giants are getting their mojo back on defense and in the running game at the right time of the season. And let's face it, the breaks do seem to be lining up in their favor, with New York in its past four games drawing the likes of the Josh Freeman-led Vikings, an injured Michael Vick and ineffective Matt Barkley in Philadelphia, a gimpy Terrelle Pryor for Oakland, and an Aaron Rodgers-less Green Bay squad. And now here comes a visit from the so-so Cowboys, in their first game since being destroyed 49-17 by New Orleans in that Week 10 Sunday Night Football massacre in the Superdome. The Cowboys beat New York 36-31 in Week 1, but that was eons ago. Dallas swooning in November and December is another staple of life in the NFC East.</p><br /><p>The Giants' six remaining opponents have a .508 winning percentage, but that stat is largely pumped up by their Week 15 home game against Seattle (10-1), the likely top seed in the NFC. New York's other games after the Dallas matchup seem winnable if the Giants stay hot: at Washington in Week 13, at San Diego in Week 14, at Detroit in Week 16 and home against Washington in Week 17.</p><br /><p>Maybe there's no explaining how New York is still alive in the playoff race. Maybe it's just an occurrence to be gawked at, like a freak show at the fair, or a 10-car pile-up on the highway. The Giants are 4-6, have legitimate playoff hopes, and might just have the division within their grasp after the franchise's worst start since 1976. The only thing we know for sure is it's mid-November, and the NFC East is getting weird again.</p><br /></div>

<div id="yom-ad-LREC" class="yom-ad yom-ad-LREC"> <center> <div id="yom-ad-LREC-iframe" class="yom-ad"></div> </center> </div> <p><br></p> <p>By Kathryn Doyle</p> <div class="body-slot-mod"> <div class="yom-remote"> <div class="yom-loading" id="mediacontentrelatedstory_container"></div> </div> </div> <p>NEW YORK (Reuters Health) – Although babies who are born weighing less than 3.3 pounds face challenges early on, by adulthood many report doing well, according to a new study.</p> <p>Growing up, those kids have an increased risk of physical and mental health problems, past studies have shown. They also tend to do worse in school than their peers and have a harder time socially.</p> <p>But by the time they reach adulthood, many seem to have left those problems behind.</p> <p>“It looks to us as if, despite many challenges early on and some functional issues, like myopia (nearsightedness), they do catch up,” Dr. Brian A. Darlow said.</p> <p>He led the study at the University of Otago, Christchurch in New Zealand.</p> <p>Darlow and his coauthors checked in with 230 adults, age 22 or 23, who were born in New Zealand in 1986 at a very low birth weight – defined as less than 3.3 pounds.</p> <p>Compared to a group of 69 adults who were born at healthy weights, the formerly small babies were still about 12 pounds lighter and 1.7 inches shorter.</p> <p>They were also slightly less likely to have finished college or advanced training. More formerly small babies had lived on welfare at some point and more reported wheezing in the past year.</p> <p>Babies born very small are usually premature or have growth restriction complications, so it makes sense that they would be prone to breathing issues. Lungs are the last organ system to fully develop in the womb.</p> <p>But people in each group rated their self-esteem and satisfaction with life fairly equally. The formerly small babies were no more likely to have attention-deficit/hyperactivity disorder or obsessive compulsive disorder, according to results published in Pediatrics.</p> <p>Those participants also scored similarly on measures of behavioral problems and were no more likely to have had depression or an anxiety disorder.</p> <p>People in both groups smoked the same amount of tobacco and marijuana, on average, and fewer participants from the low birth weight group had used other illicit drugs.</p> <p>“For me the most important message is that despite differences where they existed, these young people rated their own life functioning and quality of life no different from their peers,” Darlow told Reuters Health. “They think they are doing okay so we should respect that.”</p> <p>One in ten very low birth weight babies had a moderate or severe disability of some kind at age seven or eight. That was a good predictor of which children would still be lagging behind as adults, he said.</p> <p>Darlow said he would have liked the study to include more formerly normal-weight babies for comparison.</p> <p>“What I personally find perhaps most alarming, and probably many of my colleagues would agree, is that a larger proportion of very low birth weight adults report having few or no friends,” which fits with previous studies, Eero Kajantie told Reuters Health.</p> <p>Kajantie studies premature birth and long-term health at the University of Helsinki in Finland.</p> <p>About half of study participants born at a very low weight said they had few or no friends, compared to one-third of the healthy birth weight comparisons.</p> <p>Only one or two percent of babies are born at a very low birth weight, but they make up half the population of neonatal intensive care units, according to the study.</p> <p>“It is very rewarding to see that despite earlier difficulties, the majority of premature children have completed the basic educational requirements,” Dr. Saroj Saigal said.</p> <p>Saigal studies very low birth weight infants as they grow and age into adulthood at McMaster University in Hamilton, Ontario, Canada. She was not involved with the New Zealand study.</p> <p>A significantly higher proportion of premature babies require special assistance at school, she said.
