Kupfer, a professor of psychiatry at the University of Pittsburgh, said the new manual was faced with doing the best it could with the scientific evidence available.</p><p itemprop="articleBody" style="margin: 0px 0px 1em; font-size: 1.5em; line-height: 1.467em; color: rgb(0, 0, 0); ">"The problem that we've had in dealing with the data that we've had over the five to 10 years since we began the revision process of D.S.M.-5 is a failure of our neuroscience and biology to give us the level of diagnostic criteria, a level of sensitivity and specificity that we would be able to introduce into the diagnostic manual," Dr. Kupfer said.</p><p itemprop="articleBody" style="margin: 0px 0px 1em; font-size: 1.5em; line-height: 1.467em; color: rgb(0, 0, 0); ">The creators of the D.S.M. in the 1960s and '70s "were real heroes at the time," said Dr. Steven E. Hyman, a psychiatrist and neuroscientist at the Broad Institute and a former director at the National Institute of Mental Health. "They chose a model in which all psychiatric illnesses were represented as categories discontinuous with 'normal.' But this is totally wrong in a way they couldn't have imagined. So in fact what they produced was an absolute scientific nightmare. Dr. Insel said in the blog that the National Institute of Mental Health would be "reorienting its research away from D.S.M. categories" because "patients with mental disorders deserve better." "What we're trying to do with RDoC is say actually this is a fresh way to think about it." He added that he hoped researchers would also participate in projects funded through the Obama administration's new brain initiative.</p><p itemprop="articleBody" style="margin: 0px 0px 1em; font-size: 1.5em; line-height: 1.467em; color: rgb(0, 0, 0); ">Dr. Michael First, a psychiatry professor at Columbia who edited the last edition of the manual, said, "RDoC is clearly the way of the future," although it would take years to get results that could apply to patients. In the meantime, he said, "RDoC can't do what the D.S.M. does. The D.S.M. is what clinicians use. For years, health officials have told parents not to share utensils with their babies or clean their pacifiers by putting them in their mouths, arguing that the practice spreads harmful germs between parent and child. But new research may turn that thinking on its head.<br /><br><br /><br>In a study published Monday in the journal Pediatrics, scientists report that infants whose parents sucked on their pacifiers to clean them developed fewer allergies than children whose parents typically rinsed or boiled them. They also had lower rates of eczema, fewer signs of asthma and smaller amounts of a type of white blood cell that rises in response to allergies and other disorders.<br /><br><br /><br>The findings add to growing evidence that some degree of exposure to germs at an early age benefits children, and that microbial deprivation might backfire, preventing the immune system from developing a tolerance to trivial threats.<br /><br><br /><br>The study, carried out in Sweden, could not prove that the pacifiers laden with parents' saliva were the direct cause of the reduced allergies. The practice may be a marker for parents who are generally more relaxed about shielding their children from dirt and germs, said Dr. William Schaffner, an infectious diseases expert at Vanderbilt University who was not involved in the research.<br /><br><br /><br>"It's a very interesting study that adds to this idea that a certain kind of interaction with the microbial environment is actually a good thing for infants and children," he said. "I wonder if the parents that cleaned the pacifiers orally were just more accepting of the old saying that you've got to eat a peck of dirt. Maybe they just had a less 'disinfected' environment in their homes."<br /><br><br /><br>Studies show that the microbial world in which a child is reared plays a role in allergy development, seemingly from birth. Babies delivered vaginallyaccumulate markedly different bacteria on their skin and in their guts than babies delivered by Caesarean section, and that in turn has been linked in studies to a lower risk of hay fever, asthma and food allergies. But whether a mother who puts a child's pacifier in her mouth or feeds the child with her own spoon might be providing similar protection is something that had not been closely studied, said Dr. Bill Hesselmar, the lead author of the study.<br /><br><br /><br>In fact, health officials routinely discourage such habits, saying they promote tooth decay by transferring cavity-causing bacteria from a parent's mouth to the child's. In February, the New York City health department started a subway ad campaign warning parents of the risk. "Don't share utensils or bites of food with your baby," the ads say. "Use water, not your mouth, to clean off a pacifier."<br /><br><br /><br>In the new study, doctors at the University of Gothenburg and elsewhere followed a group of about 180 children from birth. The children were examined regularly by a pediatric allergist, and their parents were instructed to keep diaries recording details about food introduction, weaning and other significant events.<br /><br><br /><br>By the age of 18 months, about a quarter of the children had eczema, and 5 percent had asthma. Those whose parents reported at least occasionally cleaning their children's pacifiers by sucking them were significantly less likely to develop the conditions — particularly eczema — and blood tests showed that they had lower levels of a type of immune cell associated with allergies. Analyses of the children's saliva also showed patterns that suggested the practice had altered the kinds of microbes in their mouths.<br /><br><br /><br>The researchers then looked to see if the method of childbirth provided any additional protection.<br /><br><br /><br>It did. The children who were delivered through Caesarean section and whose pacifiers were rinsed or boiled had the highest prevalence of eczema, nearly 55 percent. It was so nice today.  I got out for two walks and I also cycled down to meet my sister for lunch.  <br /><br />I adore my bike it is an upright Dutch bike.  The brand is "Dutchie" and I bought it online from an English company. I’m trying really hard to be happy on this Tuesday, but man…<br /><br />Some of my exes are really gnawing at the edges of my patience these days.<br /><br />Quasi Ex-wife or ex girlfriends from my past, you ask?<br /><br />Nooooooooooooo.<br /><br />I am talking about ex-smokers, ex-drinkers, and self proclaimed, ex-food addicts.  Oy Vey!!  They are such an incredibly loathsome and insufferable lot.<br /><br />First up on the wheel of annoyance, ex-smokers.  Do I know that I shouldn’t smoke?  Yes.  Do I know it’s bad for me?  Yes.  Do I know it ultimately killed my dad and will me?  Yes.<br /><br />Do I need to hear all that on a daily basis from ex-smoking friends and ex-smokers whom I barely know?<br /><br />Oh hell no, and Jesus Smoking Christ…!!<br /><br />Their non-laconic diatribes about the evils of smoking don’t make me want to quit; their anti-tobacco pontifications make we want to fire up a Newport Red 100 so I have something to do with my hands which prevents me from punching them in their jabbering jaws of self-righteousness!!<br /><br />Some go wayyyyyyy overboard.  I have heard thie following more than once…<br /><br /><i>“We’d like to cook out more often during the summer, but our neighbor smokes in his backyard and we can smell it.  It just ruins everything.”</i><br /><br />Hmmmmm…A guy.  In his own backyard.  With a slowly burning Philip Morris product hanging from his lip…is ruining your weekend joy of cooking a piece of steroid-riddled red meat as you inhale the scented, seasonal, and life preserving fumes of Kingsford Charcoal!!?  Enough!!<br /><br />Next up on the kaleidoscope hate?  Ex-Drinkers…<br /><br />Drinking leads to no good.  Alcohol is nothing more than empty calories and heartache.  Maybe if your life weren’t so empty, you wouldn’t drink.  Ever heard any of that from an ex-imbiber? How Colonoscopies Are Like Home Renovations - NYTimes.com

It's a law of nature: Everyone who undertakes major home renovations ends up loathing their contractor. When I was recently redoing my kitchen and bathroom, I finally figured out why. It has nothing to do with the contractor's honesty, quality of work, punctuality or the mess they make. It's about behavioral economics and human psychology — in particular, the unusual way that we assess pain.<br /><br><br /><br>In the early '90s, the psychologist Daniel Kahneman and his colleagues did a series of experiments that revealed how people remembered the pain of a situation. In one experiment, participants held a hand in an ice-water bath (of 14 degrees Celsius) for 60 seconds — a pretty painful experience. To be precise: an 8.3 on a 10-point pain scale. In a second experiment they held their hand in the same ice-water bath for 60 seconds and then for another 30 seconds, during which the water was warmed just 1 degree.<br /><br><br /><br>This small increase had a big effect: afterward, when people were asked which experiment they would prefer to have repeated (for money), two-thirds preferred the second — the experiment that lasted longer and, therefore, had more overall pain.<br /><br><br /><br>Dr. Kahneman's conclusion was that people don't evaluate the pain of an experience by summing up the overall total. Instead, they remember the pain at the very end — and whether it got better or worse.<br /><br><br /><br>This was confirmed in 2003 by another experiment by Dr. Kahneman, who won the Nobel Prize in 2002, and his fellow researchers, concerning the pain associated with colonoscopy. The patients in the study underwent the usual procedure, but one group experienced a slight change at the end. How to put it delicately? After everything was finished, the tip of the colonoscope was left resting in the rectum for up to three minutes before being removed.<br /><br><br /><br>Afterward, when all the medications wore off, patients evaluated the pain of the procedure. Surprisingly, those who had the colonoscope in longer on average remembered less total pain. And this just wasn't a matter of self-reporting: over the next five years, they were also 18 percent more likely to return for a repeat colonoscopy — increasing the opportunity to reduce deaths from colon cancer.<br /><br><br /><br>Besides the fact that remodeling can often feel like putting your hands in an ice-water bath, what do these experiments have to do with one another?<br /><br><br /><br>The end of a remodeling job is always a terrible experience. A lot of little things need to be taken care of — some outlet doesn't work, the countertop wobbles just a little, the door doesn't lock. In my case, the three problems were a towel warmer that lacked an on-off switch, a shower that didn't work properly and a loose piece of molding.<br /><br><br /><br>Repairing these types of minor problems is costly for the contractor. Sending out an electrician for one outlet or the plumber to fix one faucet is unplanned for and inefficient, and really eats into the profits. Consequently, contractors balk at doing the repairs, waiting to aggregate a bunch before fixing one. For the homeowner who has just spent what seems like a fortune on the remodeling job, each problem and delay is enormously annoying. (Indeed, my molding is still loose.)<br /><br><br /><br>As Dr. Kahneman's experiments show, the pain at the end — whether it is getting better or worse — plays a disproportionately large role in determining how we remember an experience. So the fact that this game of glitch, procrastination and evasion comes at the end of the remodeling job means that we all end up hating our contractor, even if most of the job has gone smoothly.<br /><br><br /><br>So what should contractors do?
